Leucovorin (also called folinic acid) is an active form of folate (vitamin B9). Unlike regular folic acid, it can bypass certain blocks in the body’s ability to use folate. Some children with autism have folate receptor autoantibodies (FRAAs) that prevent folate from reaching the brain — a condition sometimes called cerebral folate deficiency.
Research has shown that treatment with leucovorin may improve:
Language and communication
Social interaction and engagement
Attention and focus
Behavior and mood
Children who test positive for FRAA/FRAT are most likely to benefit, but improvements have also been reported in some children without antibodies. Testing is available in-office to help guide treatment decisions.

Leucovorin is generally well tolerated, but may cause side effects such as irritability, hyperactivity, sleep changes, or gastrointestinal upset. Rarely, seizures have been reported. For this reason, all treatment is physician-guided with careful monitoring and follow-up.
